POSITION SUMMARY:

The Social Worker acts as a liaison between the residents, family and staff to build relationships, improve communication and act as an advocate for the residents.  Ensures that the emotional and social needs of the residents are met and/or maintained on an individual basis.

SPECIFIC DUTIES AND RESPONSIBILITIES:

• Develop a preliminary and comprehensive assessment of the social service needs of each resident in Skilled Nursing including a detailed history.  Assess residents’ mental status using accepted standard means.  Complete quarterly and ongoing assessments of the residents’ ability to meet clinical requirements for Skilled Nursing.
• Coordinate family and community resources as needed to address the emotional and financial needs of the residents.
• Participate with interdisciplinary team to ensure team effort in meeting resident needs.
• Assist with MDS and related care planning, care plan meetings and behavior plans.
• Schedule families and coordinate the invitations for Care Plan Meetings.
• Participate in Medicare meetings for determinations and generate denial letter for continued stay.
• Promote and support the socialization, spiritual and emotional needs of residents through collaboration with other personnel.
• Communicate with residents and their families with respect to all aspects related to the level of care provided, planning objectives and goals for the resident.  Secure supportive services to assist residents in maintaining current level of independence.
• As needs are identified, develop and conduct support groups.
• Coordinate transfers and/or discharges to other levels of care.  Prepare and support residents and families for pending changes to level of care. 
• Attend meetings and in-services as necessary.
