SpiriTrust Lutheran is now hiring a Full Time Day Nurse Practitioner for Home Care & Hospice Franklin County.

Education/Training: Graduate of certified or accredited Nurse Practitioner program. Must have and maintain current, active PA RN license, a current nurse practitioner certification in Pennsylvania; must have an active national CRNP Specialty certificate (Adult-Gerontology Primary Care or Family NP); must have active PA Prescriptive Authority; must attain and maintain CPR certification and US DEA registration. Upon hire, must have a collaborative agreement with a primary and secondary physician through SpiriTrust Lutheran LIFE available for inspection.

Experience: Two (2) years of experience as a nurse practitioner; one year of experience working with a frail or elderly population.

Eligibility for Employment: Must meet eligibility requirements to work in a Medicare/Medicaid environment including criminal background and other eligibility clearances, clearance for communicable diseases and all required immunization must be current.

Essential Duties:

  • Applies advanced knowledge and clinical skills necessary to assess and manage the healthcare of chronically ill geriatric participants in the LIFE program.
  • Conducts comprehensive health histories, physical examinations and periodic assessments of participants as needed.
  • Orders and/or performs pertinent diagnostic tests; such as, but not limited to: laboratory tests, X-rays, immunizations, microscopic exams, EKGs and other diagnostic tests as deemed appropriate.
  • Participates fully as a member of the interdisciplinary team contributing suggestions and using creative problem solving to assist participants in meeting their individual goals.
  • Advocates for the welfare of the participant and interventions to assist participant(s) and caregiver(s) in maintaining the highest possible level of independence and quality of life.
  • Participates in the 24-hour care delivery and on call rotation as needed
  • Effectively maintains and utilizes collaborative relationships with Medical Director and secondary physician to assure effective medical care is administered.
