Chandler Hall, a non-profit, Quaker facility, provides high-quality care for older adults in Newtown, Bucks County, and offers a wide spectrum of services for individuals at all stages of the aging process including Community Living, Personal Care, Residential Hospice, Home Hospice, Independent Living, Respite Care, and Skilled Nursing Care.  

The Memory care activity specialist is responsible for planning and facilitating all recreational activities for residents in Memory Care.

General schedule will be day shift plus rotating weekends.

  • Plans, organizes, facilitates and implements life enrichment programs for residents based on individual assessments to determine interests and needs for plan of care in Memory Care.
  • Plan and submit requests for materials and supplies.
  • Creates and maintains relationships with area community resources and volunteers.
  • Monitors attendance records for all programs and activities. Provides proper documentation needed.
  • Attends and reports on activities and resident council in monthly QAPI meetings with both Skilled Nursing and Personal teams.
  • Oversees Memory Care calendar and ensures programs are varying month to month.
  • Participation in Care Conferences as needed.
  • Ensures the completion of the RASP for Personal Care.
  • Lead resident council committee and complete minutes and communicate outcomes to Administrator.
  • Remains knowledgeable and keeps abreast of changes in the activities field by researching the latest methods, techniques and skills needed to enhance resident programs.
  • Reviews and supports all plans of correction for any deficiencies noted during survey inspections.
  • Provide education to new hires and existing staff on best practices in working with the resident dementia population.
  • Responsible for seasonal decorations in the Memory Care area.
  • Keeps resident family members aware of activities and special events in Memory Care.
  • Conforms with the requirements of the compliance program by understanding and adhering to Chandler Hall's Compliance Program and Policies as they appear in the Code of Conduct and other compliance policies.  Annually acknowledges review of the Code of Conduct by signature and annually participates in corporate compliance education.  Adheres to the policies and procedures covering Privacy and Security in compliance with HIPAA regulations.

Qualifications

Education and Experience

  • Experience working in long term care
  • High School required; College Degree preferred
  • Certified Dementia Practitioner
