Friends LifeCare is seeking a Director of Wellness Initiatives who is responsible for developing and overseeing Friends Life Care’s VigR ® wellness programs while co-leading the Care Coordination Department. 

This position combines strategic wellness program development with operational oversight to enhance member satisfaction, retention and socialization. This hybrid position enables you to fulfill your responsibilities from your home office, at our Blue Bell, PA office and in the field with our members as needed.

As the wellness program leader, you will spearhead the development and implementation of VigR® initiatives including educational content and socialization opportunities across multiple platforms. You'll analyze program effectiveness through participation data and satisfaction metrics to continuously enhance our wellness offerings and socialization opportunities..

In partnership with the Senior Director of Care Coordination and Director of Care Coordination, you will oversee the Care Coordination Department's daily operations. This encompasses supervising the hiring, training, and development of Care Coordinators, reviewing care plans and assessments, and ensuring high-quality service delivery. You'll monitor member risk stratification, manage departmental budgets, and maintain compliance with service agreements and HIPAA regulations.

Your leadership will be crucial in developing strategies to maintain and improve member satisfaction based on feedback and organizational direction.

Requirements:

  • Primary location is remote based from home office, with required periodic attendance at Blue Bell, PA office for team meetings and training sessions.
  • Reliable transportation for regular travel within PA, NJ and DE is required to support in-person member visits.
  • Excellent problem-solving and crisis intervention skills.
  • Excellent organizational and time management skills.

Qualifications

  • Master’s Degree in Social Work or other Human Service or Health Care discipline such as nursing, physical or occupational therapy. 
  • A minimum of 3 years of experience in a supervisory position working in a related health care field is desired, but not necessary.
  • Must maintain credentials and state licensure as appropriate and required.
  • Familiarity with wellness programs, social isolation mitigation strategies, and caregiver support is preferred.
  • Experience working with older adults, caregivers, or individuals facing social isolation is a plus.
