Salaried: Monday-Friday day shift, including on-call rotation every 5 weeks.
Responsible for clinical care administered in Healthcare. Responsible for the program planning, organizing, implementation and evaluation of this program within the framework of a resident-focused environment and in collaboration with other departments.
Direct Reports: full-time ADON, full-time Infection Preventionist/Nurse Manager, full-time Healthcare Scheduler, two full-time Nurse Managers
Position Essential Duties/Responsibilities
•Interviews, selects, hires and evaluates appropriate and qualified team members to deliver both services and programs to residents.
•Ensures processes are in place to accurately reflect the care delivered and services provided to residents.
•Understands the impact of the MDS and accurate coordinated documentation as it applies to the overall reimbursement. Develops systems for monitoring and communicates this to all staff.
•Ensures development of systems to staff skilled nursing areas with the appropriate mix, adequately trained and competent workers to deliver care and services to the residents in a cost effective manner while maintaining quality services.
•Participates and contributes actively in discussions regarding admission, appropriate placement, transfer and discharge of residents to nursing areas.
•Collaborates with peer directors and supervisors to integrate and coordinate care to residents.
•Ensures an interdisciplinary care planning program that involves staff, residents, and families in planning care agreed upon by the resident/family and able to be provided by Landis Homes.
•Collaborates with Medical Director and Health Services Administrator to develop and monitor standards of care and practices for residents and staff.
•Maintains a positive approachable demeanor that encourages staff and residents/families input and provides feedback to them.
•Adjusts staffing accordingly based on needs and complexities of residents served.
•Develops methods of communication and consistently provides communication to staff including regularly conducted staff meetings.
•Ensures staff education on an ongoing basis beginning with orientation and then provides it periodically to ensure competency of skills and timeliness of information regarding procedures, processes, policies, rules, regulations and technology in order that staff have the necessary resources, including information, to perform their job.
•Monitors Quality Improvement program that is outcome oriented and resident focused. Uses the information from this program for improvement and evaluation.
•Ensures development, implementation and monitoring of systems that coordinate and integrate care provided to residents by various disciplines including Dining Services, Social Services, Life Enrichment, Therapy, Pastoral Services and consultant services.
•Demonstrates problem solving and conflict negotiation skills in issues of staff to staff, staff to resident, staff to family, and resident to resident, and between various departments.
•Participates actively in meetings and task forces as assigned or required including Quality Improvement, Care Planning, Behavior Management, Fall Management, Compliance and Privacy, and Continuous Quality Improvement.
•Relates well to the general public, staff, residents, clients, family members, and professional peers.
•Develops and reviews strategic plan annually and identifies action plans to achieve those objectives.
•Develops an annual budget within the framework of the organization's financial objectives. Monitors monthly expenses and is able to justify variances.
•Responsible for adhering to budgeted ppd.

•Current license as a Registered Nurse in the Commonwealth of Pennsylvania required.
•Bachelor's degree in Nursing, Gerontology, health related field, or previous experience as ADON or DON preferred.
•Prior experience or education in gerontology nursing required.
•Minimum of 2 years progressive experience in long term care required.
•Has and maintains current CPR certification.
•Knowledgeable of federal and state rules, regulations, policies and guidelines governing the skilled nursing facility, including areas of care and reimbursement.
•Proficient in use of computer and related technology required for the role.
•Good verbal, non-verbal and written communication skills required.
•Demonstrates leadership, management skills, dependability and a positive demeanor.
•Demonstrates an understanding of and respect for the values and mission of the organization and its Mennonite/Anabaptist connections.
